Bilingual Social Worker/Social Worker Assistant - ADHC
Central Adult Day Health Care Center is looking for Social Worker/Social Worker Assistant. The right candidate is compassionate, patient and willing to work with the senior population. Bilingual candidates preferred, and MUST obtain a bachelor's in Social Sciences and/or related fields (i.e. Social Work, Psychology, Sociology, etc.)
Duties includes but not limited to:
- Assess and evaluate participants to complete Individual Care Plans.
- Case management.
- Lead group or counseling sessions.
- Perform home assessment, finish flow sheet, progress notes, and/or other clinical documentations.
- Coordinate with other multidisciplinary team members if necessary.
Hours: M-F 8:00AM to 2:30PM.
Salary: $20.00 to $23.00 /hour.

- ADHC: 1 year (Preferred).
- Social Work: 1 year (Preferred).
- Case management: 1 year (Preferred).
